Change of Ownership Requirements

Livingston County Health Department must receive and review the following information prior to approving the food service facility.
 
 
  Site Review Application (Change of Ownership/Use, Re-Occupancy, Equipment Addition)
  Written Scope of Intent
  “Person In Charge” (Certified Food Safety Manager)
  Menu
  Food Sources, all food suppliers and distributors
  Scaled Diagram, identify equipment
  Equipment Specifications
  Room Finishes, floors, walls, ceilings and lighting
  Pest Control Plan
  Solid Waste Plan
  Safe Water Supply
  Safe/Adequate Sewage Disposal
 
Note:  Dependent upon the complexity and intent of the project, a complete plan review may be required.
