Livingston County Health Department must receive and review the following information prior to approving the food service facility.
□ Written Scope of Intent
□ Menu
□ Food Sources,
all food suppliers and distributors
□ Scaled Diagram,
identify equipment
□ Equipment Specifications
□ Room Finishes, floors, walls, ceilings and lighting
□ Pest Control Plan
□ Solid Waste Plan
□ Safe Water Supply
□ Safe/Adequate Sewage Disposal
Note: Dependent upon the complexity and intent of the project, a complete plan review may be required.
