Homeless Continuum of Care Committee (HCCC)

The Livingston County Homeless Continuum of Care Committee was established to address housing, which has been prioritized as one on the top community needs by members of the Human Services Collaborative Body and as a basic need by consumers. The housing continuum spans from prevention programs to emergency shelter and services to transitional and supportive housing to affordable housing. Supportive services are also essential to ensure the housing is maintained. 

The Homeless Continuum of Care is a federally mandated body that is responsible for planning and coordinating services for homeless and those at risk for homelessness. 

Workgroup Goal 

Develop and coordinate a continuum of housing and services for homeless individuals and families and those at-risk of homelessness in Livingston County.

Workgroup Objectives

1. Keep people housed through prevention of eviction and foreclosure
2. Increase the capacity to develop a continuum of housing including emergency, transitional, permanent supportive, and affordable housing
3. Ensure supportive services are available to keep people housed
4. Develop a coordinated discharge assessment and planning process
5. Ensure quality and improve outcomes through needs assessment and data collection, analysis, and evaluation
6. Increase community involvement in addressing the needs of people who are homeless or at risk of homelessness


Success of the HCCC depends on the ongoing commitment of Livingston County organizations, officials, local government and residents toward a county-wide solution for homelessness. Each member is required to demonstrate their commitment to the cause by signing a membership agreement on behalf of themselves and the organization they represent.

HCCC Subcommittees

The HCCC has a number of strong subcommittees whose work is aligned with the goals and objectives of this group.

  • Homeless Management Information Services (HMIS) - This subcommittee works to ensure the data collected by the providers is entered into the HMIS system and is clean and usable data.
  • Interagency Services Team (IST) - This subcommittee bring providers, faith based providers and case managers together to problem solve cases that have a cross system need and approach.
  • Coordinated Assessment - This subcommittee is working towards a system of one point entry for our county consumers.  This will allow them to be assisted quickly and efficiently by having the assessment identify the proper plan of service.
  • Housing Stability and Affordable Housing Task Force - This subcommittee is working with landlords and the courts to reduce the number of evictions and find ways to support stabilizing current housing situations.  Coordinates the Eviction Diversion Program
  • Community Connect - This subcommittee plans the Community Connect each year.  This community wide event brings together approximately a 1,000 individuals and connects them with resources, information, food, clothing, etc. in a one day event.  Yearly reports are attached below.
  • Homeless Awareness Week - This subcommittee pulls together homeless providers to create a week of awareness in our community.
